Rates & Insurance
We provide a sliding scale fee for clients who:
 
  • Do not have insurance, or
  • If their insurance has a high deductible that they have not met.
Insurance

Presently, we accept Preferred One, South Country Health Alliance, MA, Medicare, Medicaid, MinnesotaCare, Blue Cross Blue Shieldm and UCare.  We accept other insurance, such as HealthPartners, Cigna, Medica, etc. out of network.

Please contact your insurance carrier and get the answers to the following questions:


 
  • Is mental health care included in my insurance coverage?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
  • Do I have a co-pay?
We provide EAP services.  Please check with your Human Resources to find out if your EAP is contracted with Hope Village.

 Cancellation Policy
 
If you are unable to keep your scheduled appointment, please notify us at least 24 hours in advance.  Unfortunately, if you do not notify us at least 24 hours in advance, we will have to bill you for an entire session, because we are unable to schedule another client for the session you missed.
